Patient Portal
We use Modernizing Medicine’s EMA for dermatology.
There are two ways to log in to your patient portal. One is through the website version at: https://cascadederm.modmedapp.com/ema/Login.action
The second option is using your mobile device by downloading the APPatient app in the app store. This is where we ask patients to complete forms, pay their bills or see upcoming appointments.
If you prefer to use an app on a tablet or mobile device, the APPatient app can be downloaded in the Google Play Store (Android) or App Store (Apple) and our practice URL is: cascadederm.modmedapp.com
You can check in to your appointment up to 24 hours in advanced, completed your patient forms and pay any outstanding balances including your copayment using this app!
This portal is for your medical records and secure communication with our office. When you click on the image to the left you will be directed to a different website. Please choose “continue as patient”.
